都市部で、医療・介護難民が発生するかもしれない――。医療・介護の課題といえば、財政問題だけではない。仮に国費で何とか医療・介護の財源を支えたとしても、各地域で施設や人材が不足すれば満足なサービスは受けられない。
　医療・介護の受け入れ能力が今後逼迫してくるのは、首都圏や名古屋などの大都市圏だ。２０３０年まではすべての都道府県で７５歳以上人口が増加する反面、３０年以降は減少に転じるところが多くなる。しかし、東京都や神奈川県、埼玉県、大阪府、愛知県などの３大都市圏では、３０年を過ぎてからも７５歳以上人口は増加し続ける──。
　地方の高齢化がピークを過ぎた後も、大都市圏の医療・介護需要の増加には拍車がかかるだけでなく、財源である住民税も減少していく。
　高齢化のピークが迫る中、対策は待ったなしだ。
